Processing of Application
The ASA application form (24 K) on this website serves as the application for membership and should be submitted to the Nevada State Society of Anesthesiologists at the address shown on the home page
1.15. Applications for Membership All applications for Active, Resident and Affiliate membership shall he made on a form furnished by the Society, endorsed as provided in Section 1.16 hereof and filed with the Secretary of the Society. Such form shall contain an agreement by the applicant to support and abide by the Articles and Bylaws of the Society, and shall otherwise be as prescribed by the Secretary, subject to such instructions, if any, as the Executive Committee may issue.
Active. An application shall be submitted to the Secretary of this Society on the form titled “Application for Membership”.
Change of Status. Any member wishing to change his or her category shall submit a new application through the Secretary of this Society to the Board of Directors for its consideration.
APPROVAL OF APPLICATIONS
All applicants shall be investigated by the Membership Committee of the Board of Directors, who shall recommend acceptance or rejection to the Board of Directors. A member shall be elected by a majority vote of the Board of Directors of this Society.
Eligibility Requirement:
1.122. Active Member. A physician who engaged in the practice of the medical specialty of Anesthesiology who has a degree of Doctor of Medicine, Doctor of Osteopathy or Bachelor of Medicine; whose location of principal professional activity is in the State of Nevada; who is licensed to practice medicine in the State of Nevada (except a member of the Armed Forces who is licensed to practice medicine in at state, territory, possession or the District of Columbia of the United States of America); and who has satisfactorily completed a residency training program approved by the American Board of Anesthesiology or accredited by the Council on Medical Education of the American Medical Association.
1.14.Location of Principal Professional Activity. Unless otherwise described in these Bylaws. the term "location of principal professional activity" is defined as the place where a member performs a greater part of his professional duties, as distinguished from his place of residence or location of temporary or other professional pursuits.
Each member shall maintain a mailing address within the State of Nevada except those in military service. In the event of uncertainty or dispute as to the location of principal professional activity the matter shall be referred to the Membership Committee of this Society for a ruling, and its decision shall be final.
1.161. Active. An application for Active membership shall be endorsed by two Active members of the Society who are personally acquainted with the applicant and shall be subject to the Committee on Membership, which approval shall be necessary to complete the membership requirements.
MAINTENANCE OF OTHER MEMBERSHIPS
Required Membership of American Society of Anesthesiologists: Each active and affiliate member of this Society shall be required to maintain membership in the active and affiliate categories, respectively, of the American Society of Anesthesiologists, Inc. Active and resident members of the American Society of Anesthesiologists, Inc., upon transfer of their location of principal professional activities to the State of Nevada, shall make written application for membership in this Society within ninety days thereafter.
Failure to Maintain Another Membership. Upon receipt of written notice from the American Society of Anesthesiologists, Inc., that a member of this Society is not a member of such organization, the Secretary of this Society shall give written notice to such member that he or she will be dropped from membership in this Society unless he or she regains membership and gives evidence thereof to the Secretary within sixty days. Upon receipt of notice after the sixty day grace period from such organization that a member of this Society has been dropped or expelled from membership in such organization, the Secretary shall drop such member from the membership rolls of this Society and shall notify him thereof.
